Sanction(s)
- Disciplinary Probation through December 16, 2017.
- Develop a social media awareness campaign on hazing for the SIUE fraternity and sorority community. Must be completed by December 15, 2017.
- Hold a mandatory meeting to discuss hazing, including your national policies and SIUE policies, where a minimum of 80% of the chapter is present. Must be completed by December 15, 2017.
- Ensure all future new member classes, the new member education process, and dates are registered with the Kimmel Student Involvement Center.

Sanction(s)
- Social Suspension through May 15, 2017
- All current members must completed AlcoholEdu on-line training by January 13, 2017.
- Hold a mandatory risk management education session, where a minimum of 80% of the chapter is present. Must be completed by April 30, 2017.
- Submit two separate written progress reports.
- First report should be submitted between April 15, 2017 and May 15, 2017.
- Second report should be submitted between November 15, 2017 and December 15, 2017.

- Provide letters of apology to the following:
- Building Service Workers in the Morris University Center and Student Success Center
- Building Service workers in Dunham Hall
- Office of Admissions

Sanction(s)
- Probation through the end of spring 2015 semester.
- All members of Phi Kappa Psi must complete AlcoholEdu training provided by Counseling Services.
- Self-Imposed Sanctions
- Commitment to philanthropy/service based mixers for spring 2015 semester.
- Commitment to increase service hours beginning in spring 2015, by increasing our mandatory service hours from 5 to 10 hours per semester per active member.
- Sponsorship of a yearly educational speaker for SIUE Fraternity and Sorority Life.
- Create a party planning safety packet for all members in order to prevent similar incidents in the future ane ensure Fraternity Inusrance Policy coverage.
- Increase the use of third-party vendors for fraternity events.
