When do I have to submit FMLA paperwork if I am ill or sick?

When you are off more than three consecutive days (i.e. on your fourth day off), you may be required to complete the FMLA packet and appropriate medical certification for submission to Benefits in the Office of Human Resources. You can download the paperwork at or request Human Resources to mail/email you a copy.

Once HR has received the required documents, they will be reviewed, and a letter will be mailed to you with a copy sent to your supervisor indicating whether or not you are eligible for FMLA. The letter will also state the next date an updated medical certification is required. To remain in pay status, employees must have available accumulated sick or vacation leave.
