How do I receive the protections of FMLA?

Employees need to provide, either verbally or in writing to the supervisor or department designee, sufficient information pertaining to the reason for the absence that indicates that the employee's absence would qualify for FMLA. An employee requesting FMLA should read and complete the entire FMLA packet, have their supervisor sign the application, and request their physician to complete the appropriate medical certification. The completed forms should be sent to Benefits in Office of Human Resources.
