ALL
new, transfer, or reentering students must provide proof of protection against the diseases of Rubeola, Rubella, Mumps, Tetanus, and Diphtheria per the State of Illinois mandated requirement.

THIS REQUIREMENT MUST BE MET THE 1ST DAY OF ATTENDANCE. A $25 FINE WILL BE IMPOSED FOR FAILURE TO COMPLY. PROOF MUST BE PROVIDED TO REGISTER FOR SUBSEQUENT SEMESTERS.

Proof of immunity can be provided by sending us a copy of a signed and dated school immunization record (your high school record, military record, or child health immunization record) OR by having your family physician or health care provider complete the immunization form. You may also receive the immunization in Health Service as an enrolled student.

DOMESTIC STUDENT REQUIREMENTS:

  • Tetanus/Diphtheria/Pertussis (TD/TDAP)

    • Must be within 10 years prior to enrollment

    • Tetanus Toxoid is NOT acceptable in fulfilling this requirement.

  • Measles, Mumps and Rubella (MMR)

    • Two doses at least 28 days apart and after 12/31/67

    • Must be after 12 months of age

    • OR, a combination of the following:

      • RUBEOLA (Measles, Red Measles) - 2 doses after 12 months of age

      • MUMPS - 1 dose after 12 months of age

      • RUBELLA - 1 dose after 12 months of age

    • OR, positive titers confirming immunity (please attach lab report)


INTERNATIONAL STUDENT REQUIREMENTS :

  • All records not in English must be accompanied by a certified translation

  • Tetanus/Diphtheria/Pertussis (DPT, DTP, DT, DTaP, Td, or Tdap)

    • Three doses are required

    • One must be within 10 years prior to enrollment

    • Tetanus Toxoid is NOT acceptable in fulfilling this requirement.

  • Measles, Mumps and Rubella (MMR)

    • Two doses at least 28 days apart and after 12/31/67

    • Must be after 12 months of age

    • OR, a combination of the following:

      • RUBEOLA (Measles, Red Measles) - 2 doses after 12 months of age

      • MUMPS - 1 dose after 12 months of age

  • Tuberculosis (TB Skin Test)

    • TB Skin Test 5 months prior to entering or upon arrival to SIUE

    • OR, a negative chest X-ray (please attach X-ray report)

    • OR, a negative Quantiferon Blood Test (please attach lab report)
