THIS REQUIREMENT MUST BE MET THE 1ST DAY OF ATTENDANCE. A $25 FINE WILL BE IMPOSED FOR FAILURE TO COMPLY. PROOF MUST BE PROVIDED TO REGISTER FOR SUBSEQUENT SEMESTERS.
Proof of immunity can be provided by sending us a copy of a signed and dated school immunization record (your high school record, military record, or child health immunization record) OR by having your family physician or health care provider complete the immunization form. You may also receive the immunization in Health Service as an enrolled student.

Tetanus Toxoid is NOT acceptable in fulfilling this requirement.
Two doses at least 28 days apart and after 12/31/67
Must be after 12 months of age
OR, a combination of the following:
RUBEOLA (Measles, Red Measles) - 2 doses after 12 months of age
MUMPS - 1 dose after 12 months of age
RUBELLA - 1 dose after 12 months of age
OR, positive titers confirming immunity (please attach lab report)
