To make an appointment, complete required forms (see below), or to check immunization status or messages from Health Service, please login to cougarcare.siue.edu.
Three forms are required before your first visit only: Health History Form, Notice of Privacy Practices, and Treatment Agreement. All clinic visits require a questionnaire specific to the appointment type.
Students may also make an appointment with Health Service by calling 650-2842.

Monday - Friday, 8:00am - 4:30pm
Patient Appointments:
Monday - Friday, 8:30am - 11:30am and 1:00pm - 3:30pm
Regular Pharmacy Hours:
Monday - Friday, 8:00am - noon & 1:00pm to 4:30pm
Any unforeseen changes in these hours will be posted on the Pharmacy door.
