Why was Part 3 of my Form 1095C left blank?

There are several reasons why Part 3 may be blank:
- No one was enrolled in coverage for any month of the year
- The coverage is through a fully-insured plan
- The coverage is through COBRA
- You are a union employee and your plan is through your union
