Referring Doctors
The SIU School of Dental Medicine Implant Fellowship Program welcomes referrals from dentists seeking comprehensive implant care for their patients. Our fellows provide surgical and restorative implant treatment under the direct supervision of experienced implant faculty, utilizing evidence-based techniques and contemporary digital workflows.
Unlike many programs that focus on a single implant system, our fellows are trained to diagnose, treatment plan, and restore cases using a variety of the industry's most widely utilized implant systems. We routinely provide treatment with Straumann, Astra, ZimVie, Keystone, BioHorizons, and Southern Implants, allowing us to accommodate referring doctors' preferences whenever possible.
To refer a patient, please download and complete the Implant Referral Form using the link below. Once completed, fax the form to the number listed on the referral form. You may also upload any relevant radiographs, CBCT scans, or supporting documentation using the secure file upload link provided. Additional referral information, including implant system preference and restorative responsibilities, is included on the referral form.

When care is complete, patients are returned to your practice following completion of their implant treatment for continued comprehensive dental care.
