Co-op and Internship Forms, Documents, and Process
Student:
For ALL students at the BEGINNING of each term
- Report your Co-op or internship via the online Experiential Learning form by following the instructions below:
- Login to Cougar Jobline at the top of the "Students" page in the red bar above
- Click on "Experiential Learning" on the left side of your page
- Click on "+New Experience" and enter the required information
- "Submit for Approval"
- Both the Career Development Center and your Co-op/internship supervisor will “approve” the experience - (You do nothing here)
- The Career Development Center will then email you with more information pertaining to next steps or with a denial. Some documents may NOT be due each term. You will be advised individually.
- Risk Assumption Document - upload file in “Documents and Forms” documents section
- Review instructions for the Risk Assumption Form here
- Student Pre-Evaluation Form - complete in “Documents and Forms” forms section
- Review instructions for Student Pre-Evaluation Form here
- Co-op and Internship Program Information Form - complete in “Documents and Forms” forms section
For ALL students at the END of each term
- Complete the following Student Report Instructions:
- Login to Cougar Jobline at the top of the "Students" page in the red bar above
- Click on "Experiential Learning" on the left side of your page
- Click on your blue employer’s name for this term - this is the current record
- Click on “Documents and Forms” at the top of the page next to Details
- Scroll down to the bottom of the page, if you will see yellow “Not Completed” indicators
- Click on the ellipses for Student Report Form - then click on "Complete Form"
- Click on the blue "Save" button at the button of the form and your form has been submitted
- Complete the following Employer Evaluation Instructions:
- Continue to the Employer Evaluation, and if you see a yellow "Not Completed" indicator
- Click on the ellipses for Employer Evaluation Form
- Click “Share” then you will see your supervisor’s email appear.
- If your supervisor has changed and the form needs to go to someone else, please email Tammy Taylor so she can make the change, then you will be able to share it with the supervisor of your choice
- Click “Send”
- The button will still appear yellow and “Not Completed” until your supervisor completes it
If you are continuing in your Co-op or internship next term and have not yet reported the experience, please do so following the instructions below:
- How to create a new Experiential Learning (Co-op/Internship) Record in Cougar Jobline:
- Login to Cougar Jobline at the top of the "Students" page in the red bar above
- Click on “Experiential Learning”
- “+ New Experience” and enter the required information
- “Submit for Approval”
- Student will receive an email indicating the Co-op has been submitted for initial approval
- Additional information will follow
For INTERNATIONAL Students – Additional paperwork required
- All forms and document mentioned above plus the items below:
- Letter of Eligibility Document from International Affairs
- CPT Employment Declaration Document
- There could be additional paperwork beyond these items, and we will discuss individually

Employer:
- You will receive an email to “Approve” or “Deny” the experiential learning (Co-op or internship) record completed by the student at the beginning of each term
- You will receive an email to complete the online Employer Evaluation Form at the end of each term
It is the responsibility of the student to ensure all forms are received by the Career Development Center by posted deadline dates, including forms completed by the employer.
These are the standard Career Development Center (CDC) forms utilized for everyone. The CDC may have additional paperwork if participating in a grant, special program, or if deemed necessary for purposes of transcript recognition.
Students, please note that your employer may ask you to complete additional paperwork. Please be advised that it is very important that you carefully read all documents that are presented to you to sign at your Co-op and internship sites. While we don’t anticipate any problems arising, it is possible that you could be asked to sign documents that limit your rights in some regard.
Employers, please be advised that the SIUE Career Development Center will not advertise Co-op or internship positions or permit on-campus recruiting for the following:
- Companies that require Co-op or internship students to sign non-compete agreements. We believe that this practice with respect to students is not in keeping with our office’s mission, and ultimately is not in the best interest of our students, as it can limit their future employment opportunities, without sufficient consideration.
- Companies that require Co-op or internship students to pay any fees to participate in the experience
