Replacing Your Cougar Card
Photo identification (e.g. drivers license or other government-issued photo ID) must be presented at the ID counter in the Student Services Center, Rendleman Hall, Room 1309 before your Cougar Card can be replaced.
There is a $15 fee for replacement of a lost card. If an inactive or damaged card is surrendered at time of replacement a $5 fee will be charged.
Students, faculty and staff who want an ID with a new photo will be charged $5 with the surrender of their existing card. If no card is surrendered a $15 lost card fee will be charged.
Students, faculty and staff with name changes will be issued a new card free of charge with the surrender of their existing card. If no card is surrendered a $15 lost card fee will be charged.
