I have lost or accidentally destroyed my Form 1095C. How do I get a new one?

If you have lost or accidentally destroyed your Form 1095C, it is available through CougarNet once electronic consent has been provided. Log into CougarNet, select Employment, select Tax forms and you will see the 1095C form. You will first need to provide electronic consent under the W-2 and Electronic W-2 consent bullet, or please email us at Benefits@siue.edu or call 650-2190.
