I also received a Form 1095B from Central Management Services (CMS) or Reliance Standard Insurance. What is this form and what do I do with it?

Form 1095B is a tax form issued by fully insurance plans (such as Blue Cross/Blue Shield for self-insured plans) and provides information of health insurance coverage for an individual, your spouse/partner and dependents. This form is similar to a Form 1095C. You should keep this form with your annual tax return; you do not need to send it to the IRS.
