To make an appointment, complete required forms (see below), or to check immunization status or messages from Health Service, please login to COUGARCARE.SIUE.EDU.
Please print and complete the consent form and present it at the time of your appointment.
You may read the VIS (Vaccine Information Statement) electronically.
Three forms are required before your first visit only: Health History Form, Notice of Privacy Practices, and Treatment Agreement. All clinic visits require a questionnaire specific to the appointment type.

Any unforeseen changes in these hours will be posted on the Pharmacy door.

Immunization records are being accepted for newly enrolled students and are due the 1st day of the term. If you are non-compliant, a HOLD will be placed on your ability to register for next semester. See Immunizations for further information. A fine of $25.00 will be levied the 4th week of the term if a student remains non-compliant.
