Compliance Highlight - New electronic Biohazard Risk Assessment and Biohazardous Material Use Application

ORP has recently published new electronic versions of the Biohazard Risk Assessment form and Application for Biohazardous Material Use. If your planned activity type is covered by the SIUE Policy on Biohazardous Material Use and includes the use of biohazardous materials, then you must do a professional risk assessment and complete the Biohazard Risk Assessment form found here. After submission, the chair of the IBC will advise the applicant via email as to whether the project may proceed immediately or whether a full application must be submitted prior to beginning the project. If a full application is warranted, a link to the Application for Biohazardous Material Use will be included in the email from the IBC chair.
The deadline to submit an Application for Biohazardous Material Use to the IBC is always the 2 weeks before the second Tuesday of the month in which you would like for it to be considered.
