ORP has recently published an electronic Request for Funds for Participant Incentives application. You can access the form directly here or search for it on our forms webpage.
A Request for Funds for Participant Incentives form should be submitted if you:

1. Have active grant funding;
2. You have budgeted for the purchase of incentives on the grant; and
3.  Want to purchase incentives (this is for both human subject research and non-human subject research incentives).

Please note: if you are purchasing the incentives associated with a funded human subjects research project, you will need IRB approval before you can submit the request.

Not all incentives can be reimbursed from grant funding. The form will ask for information on your grant so ORP can determine if your grant allows for reimbursement of participant incentives.
