If your financial aid eligibility has been terminated for GPA and/or Completion Rate, please complete and return this 3 page form to the Office of Student Financial Aid. This document includes the Academic Plan that must be completed and signed by your (student’s) academic advisor.

If you have completed your first baccalaureate degree and are pursuing courses required for an initial teacher certification or recertification, not including endorsements, you may be eligible to receive student loans. This is the form you will complete and submit.
