If your financial aid eligibility has been terminated, please complete and return this form to the Office of Student Financial Aid for your eligibility to be reviewed. This document includes the Academic Plan that must be completed by the student’s academic advisor.

If your financial aid has been terminated due to excessive hours, you must submit the Grad Plan to confirm when your degree will be completed. Our specialist will determine your continued eligibility for financial aid.
