Acceptance & Matriculation Requirements

Notification of Acceptance

Once a candidate has completed the Interview/Bench Examination process, they will either receive a letter of acceptance, or a letter informing them that they have not been accepted to the program. Upon receipt of a letter of acceptance, the candidate will be required to respond that they will be enrolling in the program prior to a date noted in their acceptance letter. If a response is not received by that date, their position in the program will be given to another qualified candidate.

A non-refundable deposit of $11,000.00 USD will be required by no later than November 2019. This deposit will be applied to the fee for the Clinical Certification Course upon matriculation. If matriculation is not completed, this deposit will be forfeited.

All students in the IAPP must meet and maintain all educational, professional, and financial requirements to allow for their admission, registration, and continued enrollment at SIU SDM.

At the time of acceptance, applicants will receive instructions about submitting a notarized copy of the dental degree or diploma. If the diploma is not in English, a translation must be submitted as well. Provisional certificates are not accepted.

All acceptances are conditional and contingent upon receipt of all required documentation.

International Student Requirements to be Met Before Matriculation


Immunizations are required for all international students enrolled at SIUE. All immunization records not in English must be accompanied by a certified translation. The Office of International Affairs strongly recommends that you complete your immunization requirement before you arrive at SIUE. Immunization costs in the United States can range from $75 to $200. Additional details about required immunizations can be found on the Immunization Requirement section of the SIUE Health Services website.

International Student Requirements

  • All records not in English must be accompanied by a certified translation
  • Tetanus/Diphtheria/Pertussis (DPT, DTP, DT, DTaP, Td, or Tdap)
    • Three doses are required
    • One must be within 10 years of first date of term
    • Tetanus Toxoid is NOT acceptable in fulfilling this requirement
  • Measles, Mumps and Rubella (MMR)
    • Two doses at least 28 days apart and after 12/31/67
    • Must be after 12 months of age
    • or, a combination of the following:
      • RUBEOLA (Measles, Red Measles) - 2 doses after 12 months of age
      • MUMPS - 1 dose after 12 months of age
      • RUBELLA - 1 dose after 12 months of age
    • or, positive titers confirming immunity (please attach lab report)
  • Tuberculosis (TB Skin Test)
    • TB Skin Test 5 months prior to entering or upon arrival to SIUE
    • or, a negative chest X-ray (please attach X-ray report)
    • or, a negative Quantiferon Blood Test (please attach lab report)
  • Hepatitis B
    • We strongly advise all dental students be vaccinated with the Hepatitis B vaccine prior to beginning clinical care of dental patients. Students who are not vaccinated will continue to be at risk of acquiring Hepatitis B, which is a serious disease.Should you choose not to be vaccinated with the Hepatitis B vaccine, you will be required to sign a release form.

Criminal Background Checks and Drug Screening

An applicant’s acceptance is conditional on satisfactory results of required criminal background checks and drug screening tests. Criminal background checks and drug screening tests are required for participation in many clinical settings during enrollment at the School of Dental Medicine. The background check is administered through Certiphi Screening, Inc. and is managed through ADEA–CAAPID. Following acceptance into the IAPP, applicants will be contacted by Certiphi Screening, Inc. with instructions and fees for these services.

Financial Verification

Southern Illinois University Edwardsville complies with the United States Citizenship and Immigration Services (USCIS) regulations that govern the financial eligibility of international students. Under current regulations, international students must provide evidence of sufficient financial resources to support all their living and education expenses for one academic year. Financial requirements are updated annually.

To receive financial approval for admission and a Form I-20 (the forms necessary to get a student F visa), a student must submit a current original notarized bank letter indicating in U.S. dollars that the applicant or sponsor has adequate funds immediately available. The student also must complete a Financial Form. The form must be mailed to us with an original signature to be accepted. Copies will not fulfill the requirement.

Please note: International applicants who have applied for Permanent Residency in the U.S., but have not yet been approved, are considered international applicants but are not required to provide financial documentation. International students who are in the United States on H-1, H-4 or other visa types and who plan on remaining on that same visa are not required to provide financial verification documents.
