Dear Campus Community,
03/28-04/01-Madison County-Pfizer Vaccines-1st DoseThe Madison County Health Department is now scheduling FIRST doses of Pfizer Covid-19 vaccinations for individuals in eligible groups (see list below).
Individuals who LIVE or WORK in Illinois AND fall into one of the eligible groups.
PROOF OF ILLINOIS RESIDENCY OR ILLINOIS EMPLOYMENT IS REQUIRED. YOU WILL BE TURNED AWAY WITHOUT IT.

1B - US Postal Service employees
1B - Manufacturing employees
1B - Agricultural employees
1B - Grocery Store or Grocery Supply employees
1B - Shelter and Adult Daycare workers or participants
1B Tier 2 – Individuals ages 16-64 with underlying medical conditions that increase your risk of COVID-19 (Pfizer is the only vaccine approved for individuals ages 16 and 17.)

Please Note:
- If you have ever had a severe allergic reaction (e.g. anaphylaxis) to ANY injectable medication or vaccine you are not eligible to register for these clinics. Please contact COVIDVax@co.madison.il.us for further instructions.
- You cannot receive the COVID-19 vaccine within 14 days of any other vaccines.
- If you are sick or are COVID-19 positive, please contact our department to reschedule your appointment.
- Bring a photo ID and QR code
- Bring proof of employment, if applicable, showing you qualify in either phase 1A, phase 1B, or the expanded eligibility
- Masks are required
- Wear proper attire to receive an injection in the upper arm
MINORS MUST BE ACCOMPANIED BY A PARENT OR LEGAL GUARDIAN AT ALL TIMES.
