Reporting is an important part of keeping our campus community safe and healthy. If you are unsure of your reporting responsibilities and have been exposed or ill due to COVID-19, please select your role from the following drop-down for more information and next steps.
Stay Home. Do not come to campus at any time, for any reason. Students living in University Housing should not leave their personal living space. Students should contact Health Service at 618-650-2842/cougarcare@siue.edu or their personal health provider or to determine next steps.
Complete the COVID Self-Reporting Form - This form is routed to SIUE Health Service. Be prepared to provide the date you first began to have symptoms of COVID-19, when you last were physically at work, and the date that you were tested and received your results (if applicable).
Continue participating in virtual coursework if feeling up to it; notify faculty in face-to-face coursework that you are unable to attend.
If you hold a student employment or graduate/teaching assistant position, please notify your campus employer that you are unable to report to on-ground work.
Follow the direction of health provider, SIUE Health Service and Madison County (or other applicable) Health Department.
If you think or know you had COVID-19 WITH symptoms, you can be around others after 10 days since symptoms first appeared AND 24 hours with no fever without the use of fever-reducing medications AND other symptoms of COVID-19 are improving
If you tested positive for COVID-19 WITHOUT symptoms and you continue to have not symptoms, you can be with others after 10 days have passed since you had a positive viral test for COVID-19. If you develop symptoms after testing positive, follow the guidance above for “WITH symptoms.”
Anyone who has had close contact with someone with COVID-19 should follow IDPH and CDC Guidance.
Your identity will not be disclosed to anyone other than SIUE employees responsible for maintaining your health and safety, or Health Department officials as required. You may voluntarily share your information with whomever you choose.

Contact department chair, director, or manager to inform of your absence.

Discuss with department chair or director whether participating in virtual work is an option and whether you feel up to it.
Discuss with Human Resources applicable University benefits that may be available to access related to paid time off.

The identity of the employee should not be disclosed to any person other than Human Resources. This information should remain confidential unless the ill employee voluntarily discloses their status to co-workers.
The manager should ask the employee the following questions:
The last time the employee was in the workplace
Locations the employee visited when last in the workplace.
Discuss the following:
Whether the employee’s position allows them to work virtually
Whether the employee is well enough to work from home, if that option is made available
Discuss with Human Resources and department head or manager University benefits that may be available to access related to paid time off
The employee’s responsibility in maintaining communication with the manager or department head and Human Resources
All managers who are notified should compete a 3rd Party COVID Reporting Form. The form will route to Human Resources. The form will ask you to provide the following information:
Your name
Your phone number
Best times you can be reached
Department Chairs/Directors/Managers should be prepared to provide in the 3rd Party Reporting Form the following information provided by the employee:
Have they contacted their physician?
The last time the employee was in the workplace.
Locations visited when last in the workplace.
Positive Test or Symptomatic: If the individual has been in a space during the last seven days, Facilities Management () should be contacted so they can do the appropriate cleaning and sanitizing.
In the Morris University Center, Student Success Center, Student Fitness Center, and University Housing, please contact the appropriate staff in those facilities.
If a student discloses COVID-19 symptoms, a close contact exposure, or a COVID-19 presumptive or confirmed positive diagnosis, please advise them to stay home and not come to campus.
a. If the student is enrolled in any face-to-face coursework, is living on campus or comes to campus:
i. Advise them not to attend in-person classes, to remain in their residence hall room, and not to come to campus ii. Refer them to the STUDENT SECTION of this FAQ iii. Ask them to complete the COVID Self-Reporting Form or notify SIUE Health Service at cougarcare@siue.edu or 618-650-2842
b. If the student does not generally come to campus for any reason:
i. Advise them that they should not come to campus ii. Refer them to the STUDENT SECTION of this FAQ
c. Faculty member should work with student to determine their ability to continue coursework
d. Faculty member may need to complete the COVID 3rd Party Report. If the faculty member does not believe the student has completed the COVID Self-Reporting Form, the COVID 3rd Party Report should be completed. If the student has indicated that they have informed Health Service, or if the member is aware of another faculty member completing this form, one is not necessary
i. In order to protect student privacy, a faculty member should never disclose the identity of the student via email. Faculty member should not include information regarding the medical condition of a student in any form of written correspondence, without the student’s consent. ii. If the student is also a student employee or graduate assistant, please select that classification in the COVID 3rd Party Report. That information will route to Human Resources.

If the student is living on campus or comes to campus for any reason:
Advise them not to attend in-person classes, to remain in their residence hall room, and not to come to campus
If the student does not travel to campus for any reason:
Advise them that they cannot come to campus
Refer them to the STUDENT SECTION of this FAQ
The student who learns may need the COVID-related concern of their peer may complete the COVID 3rd Party Report. If the student does not believe their peer has completed the COVID Self-Reporting Form, the COVID 3rd Party Report should be completed. If the student has indicated that they have informed Health Service, or if they are aware that another SIUE community member is completing this form, one is not necessary.
a. In order to protect student privacy, a staff member should never disclose the identity of the student via email. Staff member should not include information regarding the medical condition of a student in any form of written correspondence, without the student’s consent.
