Self-Reporting Form
Individuals are expected to complete a Self-Reporting Form if they have been tested for COVID-19, with or without symptoms, have tested positive for COVID-19 or have been given a diagnosis of presumed positive by a health professional, have symptoms, but not yet tested positive, or have had a CLOSE contact with an individual who has been tested and diagnosed with COVID-19.

The Third-Party Reporting Form is to be utilized when a third party is informed by another SIUE Community member that they have been tested for COVID-19, with or without symptoms, have tested positive for COVID-19 or have been given a diagnosis of presumed positive by a health professional, have symptoms, but not yet tested positive, or have had a CLOSE contact with an individual who has been tested and diagnosed with COVID-19. If the individual has already reported their illness to the University it is not necessary to complete this form.
