If you're interested in getting started with our program, please fill out an initial eligibility form. After the initial eligibility form, you will receive a follow up phone interview conducted by our clinic coordinator.
After your eligibility is confirmed, a physician’s clearance is required. We will then reserve a seat for you at our information meeting in which we will discuss whether this program is right for you.
