Instructions for Completing the Request for Copy of Final Grades ReportIf you need a printed copy of your final grades for your own records, you should access CougarNet at www.siue.edu/COUGARNET to view your final grades and print a copy.If you need a formal grade report for enrollment certification or tuition reimbursement purposes, complete this form, print, and sign it. To preserve student privacy, unsigned requests cannot be honored. You must complete all sections of the form. The completed, signed form may be dropped off at the Service Center, Rendleman Hall Room 1309, faxed to (618) 650-3332, or mailed to the address listed below: Your request will be held until grades have been processed for the term. See the Grade Post Schedule on CougarNet for the date final grades will be available. Questions regarding grade reports should be directed to the Service Center at (618) 650-2080. |
