· OPT 12-Month Application - For students applying for OPT after completion of their degree
· OPT 17-Month Extension Application - For STEM degree students who are currently on OPT
Students on OPT are legally required to report certain information to SIUE International Student Services. This information includes changes to any of the following events and must be reported within 10 days of the event.
· Name, Email Address, or Residential/Mailing Address
· Employer's Name,Aaddress, and Supervisor’s Email
· Immigration Status Change
· If you leave employment before your OPT end date
