Important Reasons to Contact Benefits Staff

Employee Responsibilities

Notify Your Group Insurance Representative Immediately . . .

. . . when any of the following life changes occur and may impact your eligibility or your dependent's eligibility . . .

  • Newborn or newly acquired dependent
  • Marriage
  • Divorce, legal separation or annulment
  • Death of a spouse or dependent
  • Change in your spouse's or your dependent's employment status that affects eligibility under the plan
  • Spouse who is a dependent of yours who is provided group insurance through their employer
  • Dependent no longer meets eligibility criteria or becomes ineligible for other coverage, such as dropping out of college or gaining coverage under their own policy
  • Court order resulting in you gaining or losing custody of a dependent
  • Change in Public Aid recipient status or Medicare status

. . . when you go on and return from a leave of absence. . .

. . . when you change your home address . . .

. . . if there is a change in your Medicare status or if another plan is primary . . .

. . . if your payroll deductions do not appear to be correct  for the amount  of coverage you selected.
