
**For the 2013-2014 academic year, the Vice Chancellor of Student Affairs has suspended the residency requirement and it will not be enforced. Students do not have to fill out a waiver form.

| Cancellation form for August 1 thru start of fall semester | Completed when a student is canceling their housing contract post August 1 but prior to the start of the fall semester. Required to meet one of eight approved reasons to cancel. Form should be completed and submitted to the CHO. |
| Current Resident Cancellation Form During the Academic Year | Completed when a student is canceling their housing contract during the 9-month academic term. Required to meet one of eight approved reasons to cancel. Form should be completed and submitted to the CHO. |
| New Resident Cancellation Form prior to Cancellation Deadline | Completed when a student is canceling their housing contract prior to the cancellation deadline. Form should be completed and submitted to the CHO. |
| New Resident Cancellation Form after Cancellation Deadline | Completed when a student is canceling their housing contract prior to the start of a new term but after the cancellation deadline. Required to meet one of eight approved reasons to cancel. Form should be completed and submitted to the CHO. |
| Cancellation due to academic suspension | Completed when a student is academically suspended. Form must be completed and submitted to CHO within 2 weeks of notification of suspension. |
| Study Abroad/Disney Co-Op/Internship Cancellation | Completed when student is canceling 1 semester of their academic year contract due to study abroad, Disney co-op or internship. Must be completed and returned with single student cancellation form. |
| Family Housing Cancellation | Completed at least 30 days prior to check-out date. This form is only for family housing residents. |
| Parent/Guardian Authorization Form | Housing applicants who are not yet 18 are required to submit a parent/guardian authorization form before their application will be processed. |
| Exit Survey | This survey should be completed and returned to the CHO with any cancellation form completed from above. |
| Hours Exemption | Housing residents are required to be enrolled in 18 undergraduate credit hours over the Fall-Spring semesters or 12 graduate credit hours. If you are not enrolled in at least 9 or 6 credit hours in the Fall semester, you are required to complete this form. |
| Guidelines for Counseling Considerations Cancellation Request | The following guidelines should be followed when submitting a cancellation request due to counseling considerations. |
| Permission to Access Student Housing Records | Completed by student and returned to CHO to allow parents/guardians access to housing records, including assignments and billing. |
| Domestic Partnership Statement | Must be completed and turned in with (3) supporting documents to the Central Housing Office to be considered for domestic partnership. |
| Space Change Request | The completed space change request form should be returned in your area community director. |
| Community Funds Packet | The completed Community Funds packet should be submitted to your Community Director. |
| Event with Alcohol Application | Completed application should be submitted to either the Cougar Village Commons or Evergreen Hall office. |
| Instructions for Residents who require Nurse/Attendant | Complete a Nurse/Attendant Information Form for each nurse/attendant who will need access to your room/apartment. Submit completed form to Kathleen Gardner, Associate Director of Residence Life, SIUE Campus Box 1254. Please allow two business days for processing. |
