In order to be admitted to the University, nursing and pharmacy students must comply with the immunization requirement for all accepted students. Please see the immunization section on this site for this information.
If you have an exemption for the University, this may not apply for the nursing or pharmacy program. Call Health Service if you have any questions.
In order to begin any of the School of Nursing or School of Pharmacy programs, you must complete the following requirements. You may complete these requirements through your family physician or through Health Service.
_____ Physical Examination with notation regarding no physical limitations (dated within 1 year of starting the program)
_____ Rubella antibody titer showing immunity (copy of lab report required). If you received the MMR vaccination within the past 3 months, you are exempt from needing the titer.
_____ Varicella antibody (IGG) titer showing immunity (copy of lab report required). If you have received 2 varicella vaccinations within the last 3 months, you are exempt from needing the titer.
_____ Hepatitis B vaccination – 3 doses (Doses 1 & 2 are required for first semester clearance) OR titer showing immunity (copy of lab report required). Those students who have recently completed the Hep B series must have a titer drawn 1-2 months after the 3rd shot and the titer must show immunity.
_____ PPD/Mantoux TB skin test (within 1 year of starting clinicals) – 2 step (2 separate tests spaced 1-3 weeks apart) required for 1st year students. If documented past positive and documented negative chest x-ray, will need Signs/Symptoms TB Assessment form completed. (If you have documentation of 3 current consecutive years of negative TB skin tests, this is also acceptable). Thereafter, a 1 step TB skin test is required yearly.
NURSING:
All of the required health related records must be filed with Health Service and will be reviewed by a Health Service nurse. After all requirements have been met, you will receive a clearance form appropriate to the nursing program in which you are enrolled. The clearance form will be sent by campus mail to the School of Nursing.
PHARMACY:
The School of Pharmacy will collect your health data and issue your clearance to participate. All of the requirements can be completed in Health Service and a copy of the requirements can be sent by campus mail or picked up by the student to hand carry to the School of Pharmacy.
