Immunizations

 All new, transfer, or reentering students must provide proof of protection against the diseases of Rubeola, Rubella, Mumps, Tetanus, and Diphtheria.  This is a State of Illinois law which was enacted on August 31, 1988.  To prevent problems with registration or cancellation of classes and a $25.00 fine, the completed immunization form must be submitted to Health Service by the 1st day of the 1st term of attendance.

  Proof of immunity can be provided by your high school immunization record, military records, family physician or clinic. Please send these records with our immunization form with Part I completed.  Your physician may also complete Part II of the form.  You may also receive the immunizations when you arrive on campus.

1.Tetanus/Diphteria



A. Tetanus/Diphtheria must be within 10 years.
B. Tetanus Toxoid is NOT acceptable in fulfilling this requirement.

2.MMR (Measles, Mumps and Rubella)

RUBEOLA (Measles, Red Measles)

A. Immunization with 2 doses of live measles vaccine at least 28 days apart after January 1, 1968, and on or after your first birthday, OR
B. Disease confirmed by physician records (requires physician signature), OR
C. Rubeola titer confirming immunity (attach lab report).

MUMPS

A. Immunization with live virus vaccine on or after your 1st birthday, OR
B. Disease confirmed by physician records (requires physician's signature), OR
C. Mumps titer confirming immunity (attach lab report).

RUBELLA (3 day measles, German measles)

A. Immunization with live virus vaccine on or after your 1st birthday, OR
B. Rubella titer confirming immunity (attach lab report).
C. History of disease is NOT acceptable.
