I. AGE EXEMPTION -- Students born prior to January 1, 1957, are exempt.
II. MEDICAL Contraindications -- The student must present a written, signed and dated statement from a physician stating the specific vaccine(s) contraindicated and duration or medical condition that contraindicates the vaccine(s). Medical exemption forms are available from Health Service if needed or you can download and print it from download forms section.
III. PREGNANCY OR SUSPECTED PREGNANCY -- The student must present a signed and dated statement from a physician stating such. The Estimated Date of Delivery must be indicated.
IV. RELIGIOUS EXEMPTION -- The student must present a written, signed and dated statement by the student (or parent/guardian if the student is under the age of 18) describing his/her objection to immunizations based upon bona fide religious beliefs or practice. Religious exemption forms are available from Health Service if needed or you can download and print it from download forms section..
NOTE: Anyone with a vaccine exemption may be excluded from SIUE in the event of a disease outbreak in accordance with public health recommendations.
