Job Description/Responsibilities
This position has an assigned caseload in an assigned center/program option and is responsible for providing direct services to families based upon needs.  The Community Worker assists families in creating family partnership goals and monitors progress.  S/he maintains a minimum of three contracts per month and as needed.  This position accesses community resources, makes referrals and follows up on outcomes.  S/he obtains consent forms documenting health requirements.  S/he completes various case management activities. The Community Worker may be assigned other related tasks in support of the department’s mission and goals. 

Minimum Qualifications

  1. one year of experience (paid or unpaid) in work related to the job to be performed       

Preferred Qualifications

  1. Associate’s degree in social work or human services.

$1,819 per month
