Child & Family Connections
Will help families with children between birth and age three obtain developmental evaluations and assessments. Eligibility for services will be determined by evaluations. If eligible, an Individualized Family Service Plan (IFSP) will be developed to help the children learn, grow, and receive needed services.

Family Services Clinic /St. Elizabeth’s Hospital.
Full range of services for children through adults, internal medicine, family practice, and pediatrics. Also provides immunization clinics for 0-18 year old children (M-F, 2:00-4:00). Children who are patients of Koch Clinic and under 2 years old must have an appointment for the immunization clinic. If the child is coming from another provider, the parent must have an immunization record or a prescription from the child’s doctor.

Services include: Temporary Assistance to Needy Families (TANF); Aid to the Aged, Blind, or Disabled: Food Stamps; Aid to Medically Indigent; Project Chance; and a crisis assistance program. Expedited Food Stamp Program provides immediate interview. Helps parent of a child get child support from the absent parent. Must go through the public Aid/DHS office before the State’s Attorney will file.

A state program that offers health care coverage to children and pregnant women and help in paying premiums of employer-sponsored or private insurance plans. Illinois residents through age 18 and pregnant women are eligible if they meet income guidelines. Income requirements are up to 200 percent of the federal poverty level.

Legal assistance for families at or below 125% of poverty level.
