The School participates in the American Association of Dental School Application Service (AADSAS). Applications are available in the following locations:
The formal application consists of the material processed through AADSAS and the required $20 fee, payable to Southern Illinois University, which is to be submitted directly to the School at the same time the applicant returns the completed application form to AADSAS.
The application fee is neither refundable nor applicable toward tuition. It is a standard processing charge resulting from the detailed evaluation procedures required in the consideration of applications.

Timetable
Applications for admission may be submitted between June 1st of the year preceding matriculation and the following February 1st. APPLICATIONS MUST BE RECEIVED BY AADSAS NO LATER THAN FEBRUARY 1ST in order for applicants to be considered for admission to the following year's class. Early application is to the individual's advantage and is strongly encouraged by the School of Dental Medicine.
Dental Admission Test
It is mandatory for all applicants to participate in this test. There are no exceptions permitted. The test must be taken within three years prior to actual matriculation. Inquiries related to the DAT should be made to the Office of Admissions.

Selection Factors
Each applicant is evaluated in terms of their composite scholastic record, calculation of the required science courses' credit point average, quality of preprofessional education, performance on the Dental Admission Test, recommendations, and other elements as determined by the school.
Notification
According to an agreement between dental schools that is stated in the American Association of Dental School Admission Service, December 1st is the earliest date an applicant may be notified of acceptance to a dental school, including the School of Dental Medicine. Each applicant will receive prompt notification after final action has been taken pertinent to the application.
Advanced Deposit
Each applicant accepted for admission into the School of Dental Medicine is required to submit a nonrefundable $80 deposit which is applied towards the first tuition and fees assessment. Checks, made payable to SIU, should be mailed to the Office of Admissions and Records.
Voluntary Academic Status Modification
Advance Placement
Any student who has had previous education in any course listed in the curriculum may request advanced placement. The request, stating reasons and qualifications, is to be submitted in writing to the Instructor of Record at least thirty (30) days prior to the initiation of the appropriate semester. The Instructor of Record shall notify SPAC (Student and Progress and Awards Committee) and the Dean of his/her decision regarding the petition. SPAC shall then make a recommendation regarding class standing to the Dean. The Dean shall notify the applicant in writing of his/her decision. Any related requirements or limitations recommended by the department or SPAC, including proficiency examinations, shall be included in the notification.
Proficiency Examination
Students must petition the Instructor of Record directly to request a proficiency examination. The Instructor of Record shall determine whether a proficiency examination will be granted. If a student passes a proficiency examination administered by the Instructor of Record for the requested course, credits and grade points shall be granted according to the grade achieved, as follows:
If a student receives a grade of "A", "B", or "C" on a proficiency examination, the record will show the name of the course, the hours of credit granted, and the grade earned. A notation indicating that credit was granted by proficiency examination will be included in the record. The grade earned will be computed in the cumulative grade point average.
If a student receives a grade of "D" or "F" on a proficiency examination, no grade is given, no credit is received and the record will show nothing regarding the proficiency examination. However, the proficiency examination grade shall be filed in the student's folder for reference purposes. No re-examination shall be allowed.
