Admissions Information

Application Deadline
Applications are processed using the Postdoctoral Application Support Service (PASS) of the American Association of Dental Schools and the Dental Matching Program (MATCH). PASS applications must be submitted by September 15 for the class that commences the following July 1.  Any vacant positions following the announcement of the MATCH results will be filled on an individual basis.

Application Requirements

1. The applicant must have completed or be in the process of completing a dental degree program accredited by the American or Canadian Dental Association.
2. Applicant must be eligible for Illinois permanent residency status.
3. The applicant must complete the attached application .

Please be sure to include the following:

a. Photocopy of dental school diploma (if available)
b. Official transcript forwarded by college(s)
c. Official transcript forwarded by dental school
d. Three (3) letters of recommendation
e. National Board Scores Part I
f. National Board Scores Part II (if available)
g. Small photograph (2x2)
