DUE DATE: February 15, 2013 for Fall 2013 applicants. Please send all application materials directly to Office of Graduate Admissions, Rendlemen Hall, Box 1047, Southern Illinois University Edwardsville, IL 62026-1047. DO NOT send any materials to the Social Work Department.

Graduate School online application or paper Graduate Degree-Seeking Admission Application (This step must be completed and fee paid. If you applied on-line, you are able to check your status via CougarNet (the CNet icon on the SIUE main page www.siue.edu using the same pin you used when you applied online).

Downloadable application form for Graduate Study in Social Work, Transcript Request Form, and Letter of Recommendation Form (This step must be completed. IMPORTANT! Please note and advise your recommenders, the COMPLETE 3-PAGE FORM must accompany the actual letter of recommendation or we cannot accept the recommendation. The first page MUST be filled out by you, and then sent with the remaining pages to the recommender who needs to fill out the remaining two pages and send it along with the letter to the graduate office. Three letters/forms are required via REGULAR MAIL. One strong letter from a Supervisor - a social services setting supervisor would carry the most weight - One strong letter from an UG faculty member and One other strong letter. Exceptions to this must be approved by the Program Director @ 618-650-3104).
