Each student is required to submit three forms at the time of application: Student Application Form and Essay; Medical Self-Assessment and Release Form; and Participation Agreement Form. Click on each link below to retrieve a PDF version of these documents. Please either type or print legibly.
Be certain that you have filled out each form in its entirety and all requisite signatures have been acquired and dated. The completed applications should be submitted in hard copy to Dr. Lucian Stone (Campus Mailbox 1433) along with your Deposit Check ($200.00) made out to: ‘Southern Illinois University Edwardsville.’
*Students will be notified by Dr. Stone about their acceptance or non-admittance into the program by January 1, 2010 (for those students making the December 15, 2009 deadline).
• PASSPORT: Every student must have a valid U.S. Passport or passport from their country of legal citizenship. For information on acquiring a U.S. Passport, please visit the Department of State Passport Bureau. Please note: You should apply for your passport immediately; the application process for a passport can take anywhere from one to three months.
• ISIC: All SIUE students participating in a study abroad program must acquire an International Student Identity Card (ISIC). These ID cards include important benefits, including traveler’s insurance. They can be obtained at the SIUE Office of Study Abroad on the 2nd Floor of the Morris University Center (MUC). There is a $22.00 application fee for the ISIC cards payable to the Office of Study Abroad.
