All students with an advising appointment must fill out the mandatory advisement review form prior to the appointment.
It is imperative for each student to complete Course Request Forms for each semester. Please type the needed information in the fill in form, print, sign and return it to the PAPA office by e-mail, fax, mail or hand delivery. Please first save the form to computer, then fill out form, and e-mail the completed form as an attachment to email@example.com. It takes approximately 2 weeks to process these forms. Completing these forms DO NOT guarantee placement within the specified classes!
After filling out the pdf form please save it onto your computer then send the form as an attachment to firstname.lastname@example.org

Please Read the instructions for completing the Comprehensive Registration Form since the only method of submitting this document to the PAPA Department is through EMAIL!
You may take the exam if you have completed or are currently enrolled in 30 of the 39 semester hours and must have a 3.0 GPA in both the MPA core and MPA elective courses to take the exam. Students who are currently not enrolled in classes MUST enroll in University 500 in order to take the exam. Submit your registration form electronically only. If you cancel your registration you must submit a new form electronically. Be prepared when you take the exam.
IF FOR ANY REASON YOU ARE NOT ABLE TO SIT FOR THE EXAM AND MUST CANCEL TAKING YOUR TEST, IT IS MANDATORY FOR YOU TO SUBMIT ANOTHER REGISTRATION FORM!
STUDENTS MUST SUBMIT GRADUATION APPLICATION BY THE FIRST DAY OF THE SEMESTER IN WHICH THEY WILL GRADUATE!
Students may also apply for graduation through their CougarNet account.
