ATTENTION!!

All students with an advising appointment must fill out the mandatory advisement review form prior to the appointment.

It is imperative for each student to complete Course Request Forms for each semester.  Please type the needed information in the fill in form, print, sign and return it to the PAPA office by e-mail, fax, mail or hand delivery. It may take approximately 2 weeks to process these forms.  Completing these forms DO NOT guarantee placement within the specified classes! 

After filling out the pdf form please save it onto your computer then send the form as an attachment to mpaprogram@siue.edu
