___  Type block measures 30 x 50.5 picas (5 x 8 3/8 inches or 12 1/2 x 21 1/4
     centimeters).  Please include running heads when measuring text.  DO NOT 
     include page numbers at the bottom of a page in the measurement.  NOTE 
     THAT ON THE FIRST PAGE OF THE TEXT, INCLUDING THE TITLE, SHOULD MEASURE 
     ONLY 39 picas (6 1/2 inches or 16 1/4 centimeters) IN LENGTH.
