If you would like to participate in the Conference please send an email to  H. Garth Dales (garth@maths.leeds.ac.uk) and to Andrzej Sołtysiak  (asoltys@amu.edu.pl) with the following information:

• Full name
• email and telephone number
• Institution
• Academic position
• Expected days of arrival and departure
• Preferred accommodation (private or shared room)
• Do you request financial support to defray costs of accommodation and food - if so, please give a brief reason
• Do you request financial support to waive the registration fee - if so, please give a brief reason
• Do you want to present a talk? If so, please email a copy of an abstract of your talk
• For graduate students - please ask your graduate advisor or another well-established mathematician for a letter of recommendation to be emailed separately to the same two address.

Because of the limited numbers of rooms at the Center we may not be able to accept all applications.

If your application is accepted you will be asked to pay your registration fee by money transfer to the Stefan Banach International Mathematical Center bank account (Bedlewo meetings). The account numbers are given at: http://www.impan.gov.pl/BC/accounts.html. Please indicate that the transfer is for BA2009. You can also pay for your accommodation at the same time; this will simplify your registration process after your arrival at the Center.
The Center does not currently have a secure interactive web page that could be used for credit card payments.

To pay for the registration and/or accommodation with a credit card send a fax to: (48) 22 629-39-97 (48 is the country code for Poland) with the following information:

1. First and last name
2. Credit card number; specify visa or MasterCard
3. Expiration date
4. Exact amount to be charged
5. Indicate that the payment is for the registration and/or accommodation for the Banach Algebras Conference
6. Provide a complete postal address for mailing a receipt
7. Provide fax or email number if you wish to receive a quick confirmation
8. Sign the form
 

It is hoped that decisions on acceptance at the conference and allocation of funding will be made by 15 April 2009. Applications received after that date will only be considered in the unlikely event that there are still rooms available.
