ENROLLMENT INSTRUCTIONS
OFF-CAMPUS GRADUATE CREDIT COURSES
ADMISSION AND REGISTRATION
Complete the attached Registration Form. If you have never attended SIUE,
or if you have not attended within the past three terms, YOU MUST ALSO
APPLY FOR ADMISSION TO SIUE. The Graduate School charges a $30 application
fee, which must accompany the application form.

OFFICIAL TRANSCRIPTS FROM YOUR PREVIOUS INSTITUTION FROM WHICH A DEGREE
WAS AWARDED ARE REQUIRED. Request that your transcripts be mailed directly
from your college(s) to:
GRADUATE ADMISSIONS, SIUE, BOX 1046, EDWARDSVILLE, IL 62026-1084.
IMMUNIZATION
Please note that you will be required to comply with the State of Illinois'
immunization requirement. Call 618/650-2843 for information and to request
the Immunization Form, or click Health
Service.
TUITION AND FEES
The following charges are for in-state residents. Rates may vary for out-of-state
residents or for special courses.

ON-CAMPUS AND OFF-CAMPUS COMBINED
You will be assessed tuition and fees for on-campus course, plus tuition
and applicable fees for off-campus courses.
PAYMENT
Payment may be made by MasterCard, VISA, Discover, check or money order.
SCHOLARSHIPS
Students with SIUE Cooperating Teachers Scholarship, or who are SIUE employees,
pay only fees. SIUC and SIU Medical School employees do not pay tuition
or fees. SCHOLARSHIP FORMAND/OR EMPLOYEE DOCUMENTATION MUST BE INCLUDED
WITH YOUR REGISTRATION FORM. For information about any other student financial
aid, please call this office, or contact the Office of Student Financial
Aid directly at 618/650-3880.

DIRECTORY INFORMATION
Under the Public Law 93-380, some type of student information may be made
available to the general public, unless the student has filed written
objection to this release. Directory information will also be included
in the published SIUE student telephone directory. Directory information
includes name, address, telephone number, email address, dates of attendance,
field of study, and other academic information. If you wish to file a
written objection, ask your registrar for the form.
TEXTBOOKS
Textbooks must be purchased. Books will be sold on-site at the first class
meeting. After the first class meeting books are available at the SIUE
Bookstore on the main campus, or by mail. Call 618/650-3210 for more information.

LIBRARY PRIVILEGES
Off-campus students may use all facilities of Lovejoy Library. You may
access the library using the library link.
GRADES
SIUE does not mail you a grade report. You may check your grades via CougarNet.
To order a hard copy of your grade go to the Service
Center page for the form.
DROPS AND REFUND
Drop/Refund requests must be made in writing to the Office of Educational
Outreach. Do not just stop attending class without officially dropping
the class or withdrawing from the University. You will be liable for payment
of tuition and fees for classes that you do not officially drop.

Deadlines for weekend, special format, and short term classes differ
from those listed above. For information please contact the Service Center

The registration policy and the tuition and fee refund deadline dates
differ for the School of Dental Medicine. For information please contact
